Click on the button:

then click on the CLUB button (see the red arrow in the following image).

Click on the button indicated by the yellow arrow:

A message will be shown: "Are you sure you want to leave the Club?".
Click on Yes.

The Club Leader will be advised of your leave by an in-game notification.
